USMLE Roadmap for IMGs

This guide is structured based on where you are right now in your medical career to pinpoint what you need to do next in your USMLE journey.

Stage 1: Preclinical Studies & Early CV Building (Years 1-3)
Build a strong knowledge foundation while gathering essential profile-builders for your residency application.

Knowledge Foundation

  • Focus on understanding concepts and building your basic science knowledge. Use Anki for spaced repetition from day one.

Academic Excellence

  • Doing well in your local medical school exams is an asset.

Extracurricular Strategy

  • Invest time in attending conferences, starting research projects, and pursuing hobbies.
  • Consider seeking research opportunities in the US early on; these experiences are a major asset and significantly strengthen your application.

Stage 2: USMLE Step 1 and Final Year Focus
Clear Step 1 early to allow Step 2 CK preparation to overlap with clinical experience.

Ideal Timing for Step 1

  • Plan to take the Step 1 exam (Pass/Fail) early in your 3rd year (Jan to June).

Dedicated Prep

  • The intensive period is typically 6-8 weeks. Complete UWorld at least once and take NBME self-assessments regularly.

Final Year Focus

  • After Step 1, concentrate on studying final year medical school subjects. Use time for case presentations and local conferences to continue building your CV.

Stage 3: Clinical Rotations, Step 2 CK, and Application Prep
Obtain US clinical experience, clear Step 2 CK, and finalize all application documents.

Step 2 CK Timing

  • Ideally, finish Step 2 CK before your clerkships to maximize your performance during US rotations. Preparation requires 4 to 6 months.

Clerkships (USCE)

  • Apply for US clinical electives at least six months prior to your desired start date.
  • Plan for 3 to 4 months of rotations.

OET Exam

  • Take the OET exam while you are in the US for clerkships to complete it efficiently.

Match Goal

  • Have Step 1, Step 2 CK, OET, and your Medical School Diploma completed by August of the application year, and apply by September.
